Summary:                             

This position is responsible for the overall direction and management of the Social Work department in accordance with Federal and State requirements and facility policy.

 

Essential Responsibilities/Duties:

  • Conducts pre-admission interviews with family of resident/guest
  • Participates in admission process
  • Determines advance directive code status within 24 hours of admission
  • Schedules multidisciplinary plan of care/discharge planning meetings 
  • Conducts and documents psychosocial assessment and social history of resident/guest per COMAR regulations
  • Coordinates contracted mental health services
  • Completes the face sheet and MDS per CMS guidelines
  • Completes MI/MR ID screens and coordinates Geriatric Evaluation Service review
  • Individualizes and implements plan of care 
  • Coordinates discharge planning with residents, families, staff and community resources
  • Serves as an advocate for residents/guests and families
  • Interviews, trains, evaluates and supervises Social Work personnel
  • Completes and implements an appropriate plan of correction in response to all Federal and State inspections
  • Ensures documentation in medical records is reflective of professional and industry standard
  • Participates in Care Plan Team Meetings, Performance Improvement Meetings and QAPI process
  • Provides crisis or supportive intervention with resident/guest and family 
  • Attends In-Service training to maintain and improve competencies
  • Other duties as assigned

 

Education/Experience:

State of Maryland Licensed Social Worker LCSW or LCSW-C preferred; however, we welcome and will consider BSW Social Work and MSW Soical Work
