Salary: Up to $85,000+
CareRite Centers is seeking a compassionate and dedicated Registered Dietitian (RD) to join our interdisciplinary team at one of our skilled nursing and rehabilitation centers in Nassau County, Long Island. The Registered Dietitian will play a vital role in assessing residents' nutritional needs, developing individualized nutrition care plans, and collaborating with the clinical team to enhance resident outcomes.
Responsibilities
- Perform comprehensive nutritional assessments and develop individualized care plans.
- Monitor residents' nutritional status, weight trends, and dietary needs.
- Collaborate with physicians, nursing, rehabilitation, and dietary services.
- Provide nutrition education to residents, families, and staff.
- Ensure compliance with CMS, state, and federal regulations.
- Maintain timely and accurate documentation in the electronic medical record (EMR).
Qualifications
- Bachelor's degree in Nutrition, Dietetics, or a related field.
- Current Registered Dietitian (RD/RDN) credential.
- New York State Dietitian/Nutritionist certification (or eligibility).
- Long-Term Care or Skilled Nursing experience preferred.
- Excellent communication, organizational, and clinical assessment skills.
